Roadmaps to Health Coaching
Roadmaps to Health Coaching provides support to individuals and/or teams that are working with their communities to improve health outcomes and health equity. Coaching is part of the County Health Rankings & Roadmaps program.
The Rankings show us that where we live matters and health is more than healthcare. The Rankings are based on the many factors that, if improved, can help make communities healthier places to live, learn, work, and play.

What does a Community Coach do?
Coaches help strengthen a partnership’s efforts to improve community health through guidance, questions, and support. Coaches customize their guidance for each community based on their assets and goals, meeting communities wherever they are in the Take Action Cycle. Sometimes coaching teams will be part of a group of communities supported by national organizations (e.g., National Association of Counties, BUILD Health Challenge Initiative, United Way Worldwide, etc.). Each partnership is different; we build approaches and plans with the communities and partners that make sense for their unique needs.
